Hydrotherapy is used extensively in the treatment of lumbar pain for a variety of causes including post surgery, soft tissue injury, arthritis, fractures and disc pathology. It is vital that a full land-based assessment be given to ascertain the degree lumbar pain so appropriate treatment can be given. Hydrotherapy can be very effective in decreasing lumbar stiffness and improving range of movement. Special care needs to be taken with acute back pain patients that they do not overstretch in the pool or do sudden uncontrolled movements. These patients enjoy the support and the warmth of the hot tub but can easily overdo their exercises if not closely monitored.
If the patient has neurological involvement affecting their condition the use and the types of Hydrotherapy must be reassessed and factored in to the treatment regimen. This may include discectomy, laminectomy or spinal fusion of one or several levels. Patients may commence Hydrotherapy as soon as they are medically stable, the suture line closed and no infection evident. This can be as early as a few days post surgery in larninectomies but more time may be required, especially with spinal fusions.
Hydrotherapy treatment goals: To allow the patient to gain confidence in walking, encourage movement of the lumbar spine, gain relaxation of the back extensor muscles and decrease pain.
